For Care Managers, Social Workers and Discharge Planners

You have a discharge date and a home that will not pass. This page is about how fast we can move and what you get for the file.

The Discharge Clock, Realistically

Day one. You call. We take the client’s town, the barrier, and your date.

Day two or three. We assess on site and send you a written scope with photographs and a fixed price.

From there. A safety-scope job is often one day. A full clearance runs two to four. We will tell you before you commit which one your client’s house is.

If your date is not reachable, you will hear that from us immediately rather than the day before discharge.

The Scope That Usually Matters

Very few of your clients need a spotless house. They need a property that passes a home safety assessment, and that list is shorter than families expect.

Clear egress from every sleeping room. A path wide enough for a walker or wheelchair between bed, bathroom and kitchen. Working smoke alarms within reach. No standing waste or contamination. Stairs and landings clear, which matters more in these old narrow staircases than almost anywhere. Nothing stacked against a heat source.

We will do exactly that and stop. It often costs a fraction of a full cleanout and is frequently the whole difference between a discharge that happens and one that slips.

  • Room-by-room completion photographs
  • Certificates of insurance on request
  • Written confirmation that specified safety conditions were met
  • A direct line to the crew lead, not a call center

Who Sends Us Referrals

Hospital discharge planners across Hartford, New Haven, Bridgeport and the community hospitals. Care managers at the CHCPE access agencies. Area Agencies on Aging. Adult Protective Services at DSS. Public health nurses. Home health agencies who cannot safely place an aide. Hospice social workers. Municipal health and housing officers. Conservators and probate appointees.

The CHCPE Angle, Honestly

Connecticut is better placed than most states here, and you will already know more about this than most families do.

CHCPE lists chore assistance among its covered services. Two caveats worth keeping in front of families: it is only available as part of a package for someone meeting the other criteria, so it is not a standalone route; and CHCPE is not an entitlement, with enrollment capped around 22,000, so a waiting list is real.

Authorization sits with you and the care plan, not with us. Detail on our CHCPE page.

Clients Who Do Not Want Us There

We arrive as workers, not as an inspection. No clipboards at the door, no assessment vocabulary. That single distinction does more for the first hour than anything else.

We do not force. If your client stops the job, it stops, and you hear it from us honestly rather than through a complaint. Where APS or probate is involved, we document exactly what was and was not completed.
